Specialist Physiotherapy Reports to Support Complex Funding Applications

  • Support needs for activities of daily living (ADLs)

  • Fatigue and endurance limitations

  • Risk factors impacting independent living

  • Justification for daily intensive supports or shared support models

Reports are structured to provide clear functional reasoning to inform SIL funding decisions.

AT Reports

Assistive Technology recommendations are based on comprehensive functional assessment and clinical reasoning.

AT reports may include:

  • Mobility equipment (manual and powered wheelchairs)

  • Transfer equipment

  • Positioning supports

  • Falls prevention equipment

  • Pressure care considerations

  • Environmental aids

Each recommendation includes functional justification outlining why the item is reasonable and necessary in relation to the participant’s disability and goals.

When Is a Report Recommended?

You may require an SDA, SIL or AT report when:

  • There is a change in functional capacity

  • A participant’s current living environment is unsafe or unsuitable

  • New equipment is required due to deterioration or progression

  • A plan reassessment is approaching

  • Existing supports no longer meet functional needs

Specialist Reports are typically funded through NDIS Capacity Building supports

​
